Electronic Single Referral Process

Wirral Community NHS Foundation Trust introduced the Electronic Single Referral Process for Wirral GPs back in February this year. It is the primary referral route for services provided by the trust.

Referrals to the Rehabilitation at Home Service

All referrals for the Rehabilitation at Home Service should be sent using the Electronic Single Referral Form in EMIS with immediate effect.

Phlebotomy Clinics

On Monday 4 September the phlebotomy clinic at Arrowe Park Hospital became an appointment only service and moved to Park Suite. Your patients can now go to any of the following clinics:

Dressings Clinics at Eastham Clinic

Changes to urgent and emergency care has resulted in the re-deployment of clinical staff from Eastham Walk-in Centre to Arrowe Park Walk-in Centre.

Central Advice & Duty Team telephone number change

The Central Advice & Duty Team (CADT) has moved to Wirral Community NHS Foundation Trust and their telephone number is changing.

From Tuesday 19 September the telephone number for the Central Advice & Duty Team will change to 0151 514 2222, option 3.

Single referral for Wirral CT services

After a busy surgery, GPs are often met with a task of filling out referral forms for services. The frustration is that once a clinical decision is made, the GP then has to face the form, finding the correct one amongst all of the 100s of forms that we have filed on our systems, wondering if is up to date and then worried about finding all of the information requested, some of which seems bizarre. We have listened and we have responded.

Chlamydia testing

Sexual Health Wirral is seeing increased attendances for emergency contraception and STI testing. Please remember to offer all patients aged 15-24 a Chlamydia test when attending your surgery, testing is easy and quick and the Sexual Health Team oversee all test results and management of those requiring treatment. Please contact the team on 0051 653 4416 for more testing supplies.

All Day Health Centre GP Practice closure

The All Day Health Centre GP practice is closing on Friday 30 September following a decision by NHS England to decommission the service. This includes the appointments service available for Wirral patients who aren't registered with the practice.

Do you have a patient requiring physiotherapy or osteopathy?

If you have a patient requiring physiotherapy or osteopathy you can refer them to WCT. Our waiting times may be much shorter than you expect from our expert Physiotherapy team who also offer Osteopathy which our Medical Director personally recommends for particular cases

Sexual Health Wirral launch new interactive website

Our Sexual Health Wirral team has launched a new interactive website sexualhealthwirral.nhs.ukThe website is full of information about looking after your sexual health, how to prevent sexually transmitted infections, how to access local help and support and encouraging a healthy, happy and safe sex-life.

Do you know about our Ophthalmology services?

Our consultant-led ophthalmology service provides highly specialised out-of-hospital care for patients aged over 16 who are showing symptoms of a complex eye condition to ensure their condition is diagnosed and treated/managed quickly to prevent long term eye problems.

Infection Prevention & Control Service awareness posters

Our Infection Prevention & Control Service provides specialist expert advice, support and training to health and social care professionals, nurseries and schools but did you know they also provide support and advice to members of the public?

Liscard Community Nursing Team

The Liscard Community Nurses have moved bases from Victoria Central Health Centre to -Water Street Clinic

Don't dipstick urine from catheters

There is no value in using this test with catheter specimens. GPs are advised to take the urine sample from the needle free port on the catheter bag with a syringe and send it directly to Microbiology.

Wirral Community NHS becomes a Foundation Trust!

Hundreds of thousands of people in Wirral and parts of Cheshire and Liverpool will have a greater say over how their health services are run, as Wirral Community NHS Trust is authorised as a Foundation Trust.

NEW Community Ophthalmology Service

Our consultant led ophthalmology service offers out-of-hospital assessment and treatment for Wirral patients over the age of 16 who have a complex eye condition.We offer a rapid referral process and will contact the patient within 48 hours of receiving a referral.

Demobilisation of the Livewell Service

From Tuesday 14 June 2016, the trust will no longer be contracted to deliver the Livewell Service across Wirral. This includes the Health Trainer Service, the Health Promotion Resource Centre and Health Action Areas including all Community Programme activities.

Developments in Sexual Health Wirral

There have been a number of positive developments in Sexual Health Wirral including: Postal Test Kits and the Link Team initiative encouraging health professionals, support workers, mentors, carers or patients themselves to get in touch if they feel that any barriers discourage them from looking after their sexual health & wellbeing by attending the service.

HIV testing

Sexual Health Wirral offer HIV testing at all of their clinics and following recent publications highlighting increased prevalence in heterosexuals, testing has increased. It is thought that one in five people living in the UK with HIV do not know they are infected, and Wirral in particular has a late diagnosis rate.

Sexual Health Wirral can be particularly busy at this time of year with people often not thinking to protect their sexual health over the festive period, resulting in a need for emergency contraception or STI screening.

Specialised Dental Service

We are delighted to announce that following a competitive tender process, Wirral Community NHS Trust has been selected as the preferred bidder to deliver Specialised Dental Services across Wirral, Cheshire West and Chester (including Vale Royal), in partnership with Bridgewater Community Healthcare NHS Trust. The new service goes live on 1 April.

New timetable: Sexual Health Wirral

Sexual Health Wirral clinic times and venues will be changing from 1 April 2015. The service will continue to offer their promise of a seven day service, to include 8.00am starts and evening clinics, ensuring everybody can access the service at a time convenient to them.

The Red Book just got better!

We're delighted to bring you the new and improved RED BOOK for parents and healthcare professionals.The book now contains information previously found in the Birth to Five book (no longer available), and reduces the resource and environmental costs associated with distribution of leaflets and photocopying.

0-19 Health and Wellbeing Service

This month marks the official launch of our new 0-19 Health and Wellbeing Service for local children, young people and families. The new service will help improve the current and future health and wellbeing of children and young people, help reduce health inequalities, respond to local need and provide universal and targeted support as required.

NHS mail faxing and SMS services

As you will be aware NHS Mail faxing and SMS services will end on 31st March 2015. Trust rated at 'Good' by CQC

Wirral Community NHS Trust's services have been rated 'good' following a comprehensive inspection by the Care Quality Commission. Inspectors said patients were consistently treated with compassion, dignity and respect, while patients themselves spoke positively of the experiences they had of services.

Prostate Cancer Nurse Service

In Wirral, more than 250 men are diagnosed with prostate cancer every year and this number is increasing.You can now refer patients to our team of specialist nurses who have the expertise and experience to support men who have been diagnosed with prostate cancer by a hospital consultant.

We're launching Let's Get Going - a new family fun after school club to support families with children aged 5-11 years with a BMI over the 91st centile to lose weight, get more active and live a healthier lifestyle. Diabetes support and education - change to referral process

We've made some changes to the referral process to our our diabetes education courses which are available to people with type 2 diabetes and those with pre diabetes.

Vestibular Rehabilitation

Dizziness is a multifaceted problem which, in many cases can be helped with physiotherapy. We offer vestibular rehabilitation for those conditions for which it is most beneficial.

Making life easier and safer

The Independent Living Centre introduces people to products and solutions which can make life easier and safer.  We have a permanent display of products and equipment giving people the opportunity to see and try equipment for themselves, and get information and advice.

Cdiff - letters to patients

The Infection Prevention & Control Service are, with immediate effect, sending letters to patients residing in the community who are found to be Clostridium difficile toxin positive.

Perfect Day events

Throughout the coming months we will be working with other local healthcare providers to participate in a number of 'Perfect Day' exercises for patients that visit the hospital Emergency Department. The purpose is to look at how health services can be improved to ensure that the public receive the 'right care, first time'.

Falls Prevention - early intervention

Around one in three adults over 65 who live at home will have at least one fall a year and the cost to the NHS is estimated at more than £2.3 billion. With an early referral to the Falls Prevention Service, we can help reduce the risk of falling for your patients particularly those who are 65 and over.

New Family Livewell Programme

The Livewell Team has launched a new rolling programme to support local children, young people and their families to lose weight, eat healthier and get more active. The fun and interactive family based programme focuses on ways for families to make small lifestyle changes one small step at a time.

Chlamydia Screening

The Chlamydia Screening Programme is still very active across Wirral, with national guidelines recommending that anybody attending your practice aged 15-24 is offered a routine Chlamydia test.

Verification of death

As from April 2014 Wirral Community Trust Registered Nursing staff will be able to verify expected deaths of patients who they have been caring for.

The Catheter Passport

Wirral Community NHS Trust and Wirral University Hospital Trust in collaboration with patients have developed a document called the Catheter Passport.

Single Point of Access or SPA

SPA acts as a gateway to facilitate and take referrals from GPs and healthcare professionals who wish to access other services for their patients in both the community and the acute trust. Clinicians are signposted to appropriate services for their patients ensuring that the journey of the patient is appropriate and effective.

Is your patient aged 15-24? Remember to offer the tests ...

Each GP Practice across Wirral participates in the National Chlamydia Screening Programme (NCSP). NCSP recommendations are that anybody aged 15-24 visiting their GP should be offered a Chlamydia screening test.

Rise in HIV

In Cheshire and Merseyside almost 1,500 people are receiving treatment for HIV and an estimated 350 more are unaware they have the condition, according to new Public Health England figures.

Preventing diabetes – education is key

There are three million people in the UK with diabetes and an estimated 850 000 undiagnosed (Diabetes UK, 2012). It is estimated that 7.7 % of the Wirral population has diabetes (diagnosed and undiagnosed) (Yorkshire and Humber Health Intelligence 2013).
