Integrated Care Coordination Teams

Our Integrated Care Coordination teams (ICCTs) are here to help manage your health and social care needs. They make sure you receive the right care, at the right time and in the right place. 

 

ICCTs are for people who have Health & Social Care needs that would benefit from a co-ordinated assessment and care planning to promote person centred care.  

 

Integrated Care Coordination Hubs across Wirral:

 

Birkenhead

South Wirral

Wallasey

West Wirral

 

ICCT umbrel

 

Services in the hubs include:

 

  • Community Nursing
  • D.A.S Services
  • Physiotherapy 
  • Occupational Therapy
  • Mental Health Services


People referred into the Integrated Care Coordination teams are allocated a Care Coordinator who is first point of contact for all their care.

If you have a patient who would benefit from Integrated Community Health and Social Care please refer them using our referral form, email This email address is being protected from spambots. You need JavaScript enabled to view it.  Monday to Friday, 8.30am - 5.00pm.

Multi-disciplinary team coordinators for each hub will ensure the right professionals work together to provide the right care, at the right time and in the right place!
