Health Matters: Nursing care where and when you need it

You don't need to be in hospital to receive nursing care. Wirral Community NHS Trust's community nurses provide nursing care and support for a wide range of health needs where you live or at a community health clinic.

In 2012/13, our community nurses carried out more than 281,500 visits.

Some patients have complex long term health conditions needing routine care, some need care and support because of a terminal illness and others may need a one-off visit or routine support following discharge from hospital.

Working closely with hospital services, social care, GPs and other local organisations our community nurses aim to help improve quality of life and help maintain your independence. The community nursing service provides care 365 days of the year, 24 hours a day.

With their support, the amount of time you have to spend in hospital can be significantly reduced. They can:

  • provide and assess for nursing care and give health advice
  •  monitor your health needs and progress
  • assess, treat and dress wounds including specialist wound care e.g. leg ulcer clinics
  • assess for appropriate equipment
  • some nurses are able to prescribe medicines
  • help organise and coordinate a wide range of health & social care services
  • provide palliative and end of life care to support patients who want to remain at home with their families
  • support with catheter and continence care
