Wirral Macmillan Integrated Specialist Palliative Care Team

 

Wirral Macmillan Integrated Specialist Palliative Care team play a key role in supporting people in the last days, week's year of life, both within the community and the hospital settings. One arm of the service is based at Wirral University Teaching Hospital NHS Foundation Trust and the other is based at St Catherine's Health Centre, Birkenhead. The team is comprised of Clinical Nurse Specialists and Allied Health Professionals who have specialised in palliative care. A consultant led team based at Wirral Hospice St John's (WHSJ) provide expert medical advice and support to the team.


Specialist palliative care is the active, total care of patients and their families with progressive, advanced disease including cancer and non-cancer diagnoses with complex symptom and / or psychosocial needs. Care is provided by a multi-professional team who have undergone recognised specialist palliative care training.


The aim is to provide physical, psychological, social and spiritual support through education and clinical advice to a wide range of professionals

 

Access

  

Accessing the service:

 

Referrals can be made via a healthcare professional ie GP or community nurse.


For patients with advanced, progressive incurable, malignant and non malignant disease who have complex physical, psychological, spiritual, and social or carer needs.


Where the current healthcare professional requires support and advice of the SPC services:

  • Where the patient or family needs are unable to be met by health professionals in the current care setting and may be met by a SPC Service. This may be via advice and support given to the referrer and not require face to face assessment of the patient with discharge back to the referrer at this juncture.
